I have prepared the following list of items that you should bring to your consultation that will assist me in getting to work immediately.
Every case is different so do not be concerned if any of the items listed do not seem applicable to your own situation or if you do not have all of the listed items. It is more important for us to meet promptly than for you to wait until you have gathered up all of the listed items. Any items that you currently do not have can always be obtained at a later date. The most important thing is that you contact me today.
- Any photographs of the vehicles involved in the accident and/or the scene of your accident if you were injured upon the property of another person or company
- Any photographs depicting the injuries that you sustained in your accident
- Any accident report or exchange of information provided to you at the accident scene identifying all parties involved in the accidents and any insurance companies who might be involved and/or a copy of any written accident report that you wrote or signed
- Names, addresses and telephone numbers of any witnesses to the accident
- Names and descriptions of any employees of the business where you were injured who either witnessed your accident or obtained a statement from you
- Names and addresses of anyone who has provided you with medical or chiropractic treatment for your injuries since the accident and the names and addresses of any facilities where you have received treatment since the accident
- Names and addresses of anyone who has provided you with medical or chiropractic care for the past five years and the names and addresses of any facility where you have received treatment for the past five years for any reason
- Your automobile insurance policy or any documents showing the types and amount of automobile insurance coverage in effect at the time of your accident
- Any group health insurance policy or HMO documents that may provide you with coverage for the injuries sustained in your accident
- The names, addresses and telephone numbers of any insurance company representatives who have contacted you since your accident
- The claim numbers that any insurance company has assigned to your accident claim
- Any letters that you have received from any insurance company or any other person or company on behalf of any other person or company involved in your accident
